TO COMPLY WITH 35ILCS 200/15-172 OF THE PROPERTY TAX CODE
The Chief County Assessment Office wishes to remind senior citizens that the deadline to file the 2023 Low-Income Senior Citizens Homestead Assessment Freeze, Department of Revenue form PTAX 340 is JULY 1, 2023. The application needs to be filed with the County Assessment Office. If you think you may qualify to file this application, but could not leave your home to obtain assistance in completing the necessary form, Volunteer Services has Outreach Staff that will come to your home to assist you.

- Senior Citizens Assessment Freeze PTAX-340 Must be signed up for the Senior Homestead Exemption.
- Senior Citizens Homestead Exemption PTAX-324 Must Apply in Person with Valid Identification. Also a PTAX-325 Request for Duplicate Delinquent Property Tax Notice needs to be filled out in person, for first time applicants
- Returning Veterans’ Homestead Exemption PTAX-341 Must provide a Certified Copy of DD214 Discharge.
- Disabled Veterans’ Standard Homestead Exemption PTAX-342 Current years percentage of “service-connected disability rating,” and a certified Copy of DD214 Discharge. Also a PTAX-325 Request for Duplicate Delinquent Property Tax Notice needs to be filled out in person, for first time applicants.
- Disabled Veterans Standard Homestead Exemption Renewal PTAX-342-R Current years percentage of “service-connected disability rating” required.
- Disabled Persons’ Homestead Exemption PTAX-343 Current years proof of disability. If 65 years or older for proof of disability would need a Class 2 Illinois Person with a Disability Identification Card from the Illinois Secretary of State’s Office. Class 2 or Class 2A qualifies for this exemption. Class 1 or 1A does not qualify. Or submit PTAX-343-A Physician’s Statement, this form must be completed by a physician. Also a PTAX-325 Request for Duplicate Delinquent Property Tax Notice needs to be filled out in person, for first time applicants.
- Disabled Persons’ Homestead Exemption PTAX-343-R Renewal Current years proof of disability required.
